What we publish
Before the Incident publishes research on serious injuries and fatalities (SIF), the critical controls that prevent them and the weak signals that come first. It is written for EHS and safety leaders in heavy industry, and for the operations leaders and boards who own the risk.
Editorial standards
How we source, check and correct our work.
- Sourcing. Figures come from named public datasets, published studies and standards, cited at source with the unit, population and period. Where we build a model, every assumption is shown.
- Methods. Each report and exhibit states how its numbers were produced and what they do and do not show. See Methods.
- Corrections. A corrections address will be published here before launch (placeholder). Errors are fixed in the piece and noted on it.

Evidence rules
- Every number carries a source, a base and a period.
- Interpretation is labeled as interpretation.
- Placeholder or illustrative data is labeled on every exhibit that uses it, and is never presented as evidence.
- We describe conditions, controls and systems. We do not name people or sites without consent, and we do not dramatize real events.
